Its much much higher than 40%. Please check for any modifiers.
Is it a 40% chance that you might have 1 guy survive?

I'm not joking, i think it got screwed up and its a 40% x 40% multipliers (16%)

we had this exact problem the last time we put the survival at 40%
doesn't have anything to do with PE.
although fwiw it seems to me that stacking two squads into an IHT is a death sentence

It is bugged, my fault for not realizing I hadn't updated the files with the 40% modifier. Right now I think it's only at 20%, which is causing a lot of destroyed squads. It should be fixed in the next patch to actually be 40%.
