Both of these bugs are fixed in the most recent version.  I hope anyway.

When did you play, because it was only released recently and thus your launcher would have needed to update first Tongue.

It's fixed.  I promise.  I tested it multiple times.  If players are dropping that can create an anomaly.  But I'm 99% sure it's fixed.  If it isn't, I'm clueless as to what it could be unless you post the scarlog.
